Mili Medical Services, a Health Service Company with Two Units

Mili Medical Services (MMS) provides services in health sector segments that are not covered otherwise. Funders, Dr. Rokai Rauofi an 8 years experienced surgeon, with 3 years’ experience in Histopathology, and Mr. Abdul Baseer a biomedical technician with 10 years’ experience, cooperate as individuals since five years.

Histopathology

Microscopic examination of body cells allows differentiating benign and malignant occurrences. The specimen can be taken by a minor intervention, like Fine Needle Aspiration (FNA), Biopsy, or during an operation. The availability of this laboratory service allows cancer identification at an early stage.

Benefit for Poor People

The regional availability of this service encourages early stage examination before severe symptoms force patients for testing abroad. Tuberculosis and cancer are often misinterpreted; histopathology guides clinician’s decision for the right therapy. Even at a final stage of cancer, the examination can prevent families from financial ruin for hopeless treatments, and encourage for palliative care.

Benefit for Women

Cytological examination of breast tumors and lesions of genital tract has minimal risks, low costs. This helps women who cannot afford diagnostics abroad.  Samples can be taken in-outpatient settings, and give results within hours. It will relief 80% from fear of breast cancer and help others for an informed decision. The expected profitability of Histopathology-tests will allow providing a percentage of services as free of charge for poor patients, while still below costs compared to examinations abroad.

Biomedical Equipment Maintenance

Medical equipment is essential for a functional health facility. International support encouraged multimillion investments for medical equipment. Loss of equipment from unprofessional maintenance is an un-solved problem. Hospitals today have stockpiles of broken medical equipment on their compounds. Even worse, they use equipment after improvised unskilled repairs, with dangerous malfunctions, leading to the maleficent results. Our medical maintenance service expects an 18 months maintenance contract for three hospitals with an international project. Stakeholders of the project consider MMS as important factor for the sustainability of their investments in hospitals.

Customer and Poor Population Benefit

Preventive maintenance extends the lifetime of medical equipment, and prevents risks. A technically skilled workshop and technical team can prevent equipment failure. This improves the reliability of regional health facilities, reducing risks for patients. The Poor have most benefit from technically sound public hospitals. Poor people cannot move to private hospitals or abroad.

Outlook

Both components have potentials for national expansion. Our individual staff contracts, can convert to service contracts between MMS and the international projects were we work, once the MMS is operative. Our employers support our approach, giving the opportunity to start with a financially reliable client. The anticipated 18 months contract, will involve the full range of biomedical services for 3 hospitals, and provides us also some help by international experts from the project, who are willing to support as mentors during our startup. MMS is already considered as follow-up service provider for donated medical equipment, after international support ends. Our approach meets the anticipated demand for Public Private Partnerships (PPP) on technical maintenance, that the Ministry of Public Health (MoPH) prepares with support of international consultants. MMS can be the first company to bring the strategic PPP concept to operative realization. The possible opportunity from our startup is to bid be the end of the year as the only experienced medical maintenance provider for an expected MoPH service contract and become the first accredited service provider for Medical Maintenance Services in Afghanistan.

Why two Units in one company?

MMS provides both units in separate business models and budget plans, to prevent confusion. The components will not cross-subsidize each other. Both components could be individual business concepts. Our close distinct cooperation over years, and our political and client related networks make it important for us, to have both units in the same company. If one of our business concepts might not convince you, we rather accept to be rejected with all the application, instead of receiving a splitting reply supporting one, but not the other approach. In the limits of our language capacity we did our best to present or concept for your consideration.
